The Elizabeth Seton Pediatric Center's Home Health Care Program is committed to changing lives one child at a time.

Children with special health needs deserve dedicated professionals who care about them as people, understand their special needs, and appreciate their unique gifts and personalities.  Our goal is to provide the services necessary to enable them to live at home and to respect and support the unique bond that exists between parent and child.

We provide comprehensive case management in order to deliver complex services to children and their families.  Most experts agree that home is the best environment for healing.  

Some of the services that we provide to children include:

  • skilled nursing visits
  • physical, occupational, speech, respiratory and language therapies 
  • social work services
  • nutritional counseling
  • home health aides, personal care aides, and homemaker services  
  • medical supplies, equipment and transportation arrangements

 

We also care for:

  • Children with multiple needs who have a complex plan of care including children with physical and mental disabilities, AIDS and complex neurological disorders

  • Children whose health status is apt to deteriorate rapidly  

  • Children whose health or functional status can be expected to stabilize or improve with the provision of home care services

  • Children with a poor prognosis for recovery  

Our Home Care Program serves children from birth to 21 years of age.  A registered nurse will visit the home to complete an intake assessment of the child's needs and coordinate care as needed. If needs can be met at home, the nurse will complete a comprehensive health assessment and work with the child's physician to develop a collaborative care plan to promote and maintain the child's health, growth, and well being.
